We would like to invite you to come along and meet the hub to learn more about our service and the dementia support we offer in Bradford.
We offer information and advice for people who are living with dementia as well as support to carers, family members and friends.
The event will be an opportunity to meet some of team and gather information for yourselves or a loved one.

How to make a referral
Referrals can be made for adults of any age who are on their dementia journey. Referrals into the service can be made through Memory Clinics, a GP/health professional or you can also self-refer on this website.
